Overview

Te Korowai Hauora o Hauraki is a not-for-profit kaupapa Maori health organisation, based in Hauraki.

Providing medical, wellness and community services in Coromandel, Paeroa, Waihi, Te Aroha and Whitianga areas, we have successfully delivered services in Hauraki since 1994.

We aim to be a leading indigenous health provider by providing high-quality, whanau-centred services in the Hauraki locality.

We are looking for a Health Coach to join our Ohu Hinengaro (primary mental health) team.

This full-time (1.0 FTE), non-clinical role will be based at a designated general practice clinic in Hauraki.

Role and Reporting

Reporting to the Hinengaro Team Lead, the Health Coach will work closely with our Whanau Health Centre team members, general practice teams, Te Whatu Ora - Mental Health & Addiction colleagues and other Te Korowai Hauora o Hauraki services to ensure whanau needs are met.

Responsibilities

Engage in an integrated Stepped Model of Care that supports whanau needs and enables them to move between services seamlessly.

Collaborate with the Ohu Hinengaro team and operate within a Mahi a Atua approach to transforming systems, processes and outcomes for whanau and addressing health inequalities for Maori.

Support whanau with service choices and improve outcomes for whanau through an evidence-based practice, including Feedback Informed Treatment where Mataora / practitioners gather real-time feedback and adjust practice accordingly.

Highly engaged and passionate about working with whanau in a kaupapa Maori environment.

Committed to achieving health equity and driving quality improvement to ensure better health outcomes.

New Zealand based applicants with eligibility to work in New Zealand will be considered.
