ProCare Fresh Minds has exciting opportunities for someone wanting a career as a Health Coach.
You would be based at the heart of primary care within the general practices based across Central Auckland.
We are keen to hear from applicants interested in a career that supports people within their community who want to make lifestyle changes to better manage their health.
This is a diverse and fulfilling new role that empowers whaiora and whanau to gain the knowledge, skills, and self-confidence to reach their own health and wellbeing goals.This includes lifestyle changes (smoking cessation, exercise); promoting self-management especially for managing long-term conditions like diabetes; and helping navigate primary care and the wider social services they need to help them succeed.This Health Coach role is part of the innovative programme, Te Tumu Waiora.ResponsibilitiesExciting and fulfilling role focused on improving the health and wellbeing of Auckland Whanau, especially Maori, Pacific, and youth.Relationship-centred – Integrated within a general practice clinic and connected to a national rollout.Supporting whaiora / whanau / groups to manage their long-term conditions and / or lifestyle challenges.Specialised training and support provided.Permanent Part Time positions : various hour options available across the Auckland city and central fringe areas, one area is St Heliers - 0.6 up to 0.8 FTE.You will bringAre self-motivated, independent, and enjoy working as part of a team dedicated to improving peoples' health and wellbeing (this includes individual patients, whanau, and group consults).
Have excellent communication skills and a positive can-do attitude.Can consistently work within scope and systematically follow the steps within the Health Coach model (training provided).
Are keen to set up and run groups for patients enrolled in the general practice e.g. walking groups, diabetes groups etc (support provided).
Are confident to work collaboratively with GPs and nurses, and other Te Tumu Waiora team members (Health Improvement Practitioner and Community Support Worker) in your clinic context.Will need to be resilient, flexible, organised, and able to create relationships with people from all walks of life.Have an understanding and sensitivity to working with all cultures.Will have good typing / IT skills as you will be required to enter clinical notes into the Patient Management System in a timely manner.Do not need a degree; however, you must desire to support whaiora and whanau to improve their health and wellbeing within general practice.This role may suit someone who has been a peer support worker, volunteer, or has lived experience supporting a whanau member with a long-term condition such as diabetes.
