This includestreatment / recovery plans developed together with the service users andfamily / whanau.
The role also involves on - going monitoring and dischargepanning.Along with case management the Social Worker also providesindividual and group work, education and providing sound clinical and culturalknowledge in the MDT discussions.
TheSocial Worker will work closely with service users / whanau and communityagencies to achieve the outcomes agreed by the multidisciplinary process.
The Social Worker will also contribute totheir own professional development activities as well as wider social workactivities within the district.The salary range for this role is $77,087 - $107,600depending on experience.About theTeam / Service / LocationMHAIDS is the mental health, addiction and intellectual disabilityservice for the Wairarapa District and Capital, Coast & Hutt ValleyDistrict, with multiple specialist facilities.
Te Whare o Matairangi is a 30-bedacute inpatient Mental Health Recovery unit providing at Wellington RegionalHospital, providing care for tangata whaiora experiencing significant mentalhealth issues.
