  • About the Role
  • Clinically support mental healthservice users / tangata whai ora during periods when they are experiencingmoderate to severe periods of psychiatric illness that require assessment,treatment and support to assist the individual and their whanau and supportsalong the recovery journey, back to primary health care.

    • Our client group is diverse,and we cover a broad-spectrum both socioeconomically and culturally.

    Specialist services also cater to specificpopulation groups e.g. Maori Mental Health, Health Pacifica, Maternal orCommunity Drug and Alcohol Services.The Social Worker is responsible forproviding clinically effective tangata whai ora assessment, treatment,intervention and goal setting with tangata whai ora.

    This includes treatment / recovery plansdeveloped together with tangata whai ora and family / whanau.

    The role alsoinvolves on - going monitoring and discharge panning.Along with case management the SocialWorker also provides individual and group work, education and providing soundclinical and cultural knowledge in the MDT discussions.

    The Social Worker will work closely with tangatawhai ora / whanau and community agencies to achieve the outcomes agreed by themultidisciplinary process.

    The SocialWorker will also contribute to their own professional development activities aswell as wider social work activities within the district.

  • About theTeam / Service Location
  • We work from a multi-disciplinary team (MDT)framework and the social worker is an important part of the MDT.

    • We work closely with tangata whai ora and whanauto meet individual needs that support recovery and progress through arehabilitation pathway that has a focus on the development of personal skillsenabling people to reintegrate into local communities and eventual discharge.

    The Social Worker works closely with thenurse keyworker for each individual to ensure actions are completed in a timelymanner - communication is an important part of each MDT members skill set.

  • About you
  • ExperiencedSocial Worker with understanding of mental health, recovery and strengths-basedmodels.

    • You need excellent communicationskills working with tangata whai ora, whanau and the wider MDT as well asexternal stakeholders.

    Your ability toplan and prioritize your case load is essential in this busy clinical setting.

  • Essential qualifications, skills and experience
  • Previous experience in working as a social worker with mental health and disability tangata whai ora in a public sector health setting would be desirable.
  • Registered Social Worker with current annual practicing certificate;
  • Current full NZ driver - s licence with ability to drive a manual and automatic car;
  • Proficiency in Microsoft Office, Word, Outlook, PowerPoint, Internet resources and e-mail;
  • A high standard of written and spoken English.Someone well-suited to the role will place ahigh value on the following :

  • A commitment and understanding of the Te Tiriti o Waitangi (and application to health) and a willingness to work positively in improving outcomes for Maori.
