Specialist services also cater to specificpopulation groups e.g. Maori Mental Health, Health Pacifica, Maternal orCommunity Drug and Alcohol Services.The Social Worker is responsible forproviding clinically effective tangata whai ora assessment, treatment,intervention and goal setting with tangata whai ora.
This includes treatment / recovery plansdeveloped together with tangata whai ora and family / whanau.
The role alsoinvolves on - going monitoring and discharge panning.Along with case management the SocialWorker also provides individual and group work, education and providing soundclinical and cultural knowledge in the MDT discussions.
The Social Worker will work closely with tangatawhai ora / whanau and community agencies to achieve the outcomes agreed by themultidisciplinary process.
The SocialWorker will also contribute to their own professional development activities aswell as wider social work activities within the district.
We work closely with tangata whai ora and whanauto meet individual needs that support recovery and progress through arehabilitation pathway that has a focus on the development of personal skillsenabling people to reintegrate into local communities and eventual discharge.
The Social Worker works closely with thenurse keyworker for each individual to ensure actions are completed in a timelymanner - communication is an important part of each MDT members skill set.
You need excellent communicationskills working with tangata whai ora, whanau and the wider MDT as well asexternal stakeholders.
Your ability toplan and prioritize your case load is essential in this busy clinical setting.
