Through integration and collaboration, we build a health system underpinned by partnership, equity, sustainability, whanau-centred care, and excellence.About the RoleWe currently have a social work position available in an inpatient medical ward at Wellington Regional Hospital.The medical service at Wellington Hospital provides assessment, diagnosis, stabilisation and treatment, and rehabilitation of patients who present for medical diagnosis and treatment.The successful applicant will assist patients and their whanau during their hospital stay to support them with the impact of their admission, treatment and discharge.

The role holder will complete psychosocial assessments with patients and their whanau and work in partnership with their multi-disciplinary colleagues as well as their nursing and medical colleagues to facilitate supportive and safe discharges from hospital.

You will respond to a range of referrals including addressing complex social issues, discharge planning and transition into Aged Residential Care.The role covers into other areas of the hospital across inpatient, outpatient and community settings as required.About the Team / Service / LocationWe are a diverse and inclusive team of 23 social workers who provide a service to patients and their whanau across inpatient, outpatient and community settings.

We complete psychosocial assessments across the age range and provide holistic social work interventions.

We encourage therapeutic social work and trauma informed practice, and value patient-centred care.

The role will be well supported by the other medical social workers and social work coordinator in the team.

You will be able to work collaboratively and effectively as part of a multi-disciplinary team and with external agencies.

Working with a diverse group of patients with significant health needs the post holder will require excellent documentation, communication and time management skills.
