Job description

Overview

Active+ is the largest New Zealand owned supplier of rehabilitation services with over 1,500 health providers across a wide range of disciplines. We have been in business since 1990 and are owned by Southern Cross Healthcare New Zealand.

Mō te Tūranga | About the Role

Active+ is seeking a Health Navigator to join the Integrated Care Pathway (ICP) team in Auckland. In this role, you’ll work with Kiritaki (clients) to access and engage in the care they need to recover and thrive. You’ll engage with clients, understand their rehabilitation needs, coordinate appointments, manage communication, and remove cultural, logistical, or systemic barriers to make their journey through the ACC ICPMSK service as seamless and effective as possible.

As part of the Navigation Team, you’ll collaborate with fellow Navigators, our national provider network, and internal teams to deliver high-quality, connected support. You’ll provide rehabilitation pathway insights to guide consistent care, develop resources to educate clients and providers, and monitor performance to ensure services meet contract and outcome requirements.

This is a full-time, permanent role based in Auckland, offering flexible hybrid working and opportunities to visit clinics across our network. If you’re driven to deliver results, passionate about client-centred care, and skilled in managing complex health journeys, this is your chance to make a real impact.

Ngā pūkenga me ngā wheako | Skills and experience

  • Proven expertise in client advocacy – navigating complex health journeys and supporting clients to achieve the best possible outcomes.
  • Deep understanding of the ACC ICPMSK model and experience working within contract frameworks to deliver quality, compliant services.
  • Culturally safe and inclusive approach with a genuine commitment to equity and respectful communication across all interactions.
  • Outstanding communication skills – able to conduct robust client conversations, discuss health histories, educate clients about programme expectations, and maintain clear, professional communication with providers, ACC, and internal teams via calls, email, and text.
  • Highly organised and detail-focused, able to manage multiple clients at different stages of their pathway.
  • Tech-savvy with experience using Microsoft 365, Excel, and practice management systems such as Gensolve.
  • Collaborative and values-driven, aligned with our principles : Care First; Better Together; Pursue Excellence.
  • Experience in healthcare or client-facing roles is an advantage, bringing insight into supporting people through their recovery journey.
  • Eligibility to work in New Zealand – New Zealand citizenship or residency is required.

Ngā mea ka taea e mātou te tuku I a koe | What we can offer you

  • Competitive Rewards : Attractive remuneration package, with an extra ‘appreciation day’ per annum, generous Christmas / New Year breaks, and long service benefits.
  • Health & Wellbeing : Funded health insurance, a comprehensive wellbeing programme, and funded online GP and EAP access.
  • Flexibility & Growth : Flexible working arrangements and well-supported learning opportunities, with professional development and access to Southern Cross Learning Hub modules including Cultural programmes.
  • Ko wai matou | Our Organisation
