Active + Health Navigator for the Integrated Care Pathway (ICP) team

Active+ is seeking a Health Navigator to join our Integrated Care Pathway (ICP) team in Auckland.
In this pivotal role, you'll work alongside Kiritaki (clients) to ensure they can access and engage in the care they need to recover and thrive.
You'll engage with our Kiritaki, develop an understanding of their rehabilitation needs, coordinate appointments, manage communication, and remove barriers—cultural, logistical, or systemic—to make their journey through the ACC ICPMSK service as seamless and effective as possible.
As part of the Navigation Team, you'll collaborate with fellow Navigators, our national provider network, and internal teams to deliver high-quality, connected support.
You'll also provide rehabilitation pathway insights to guide consistent care, develop resources to educate both clients and providers, and monitor performance to ensure services meet contract and outcome requirements.
This is a full-time, permanent role based in Auckland, offering flexible hybrid working and opportunities to visit clinics across our network.

Proven expertise in client advocacy – skilled at navigating complex health journeys and supporting clients to achieve the best possible outcomes.
Deep understanding of the ACC ICPMSK model and confidence working within contract frameworks to deliver quality, compliant services.
Culturally safe and inclusive approach, with a genuine commitment to equity and respectful communication across all interactions.
Outstanding communication skills – confident in having robust client conversations, discussing health histories, educating clients about programme expectations, and maintaining clear, professional communication with providers, ACC, and internal teams via calls, email, and text.
Highly organised and detail-focused, able to manage multiple clients at different stages of their pathway while keeping everything on track.
Tech-savvy, with experience using Microsoft 365, Excel, and practice management systems such as Gensolve.

Experience in healthcare or client-facing roles is an advantage, bringing valuable insight into supporting people through their recovery journeys.
Eligibility to work in New Zealand – New Zealand citizenship or residency is required.
