This initiative enables free patient access to essential services, supports workforce development, and fosters innovative, community-based care.About The RoleThis is a 0.5 FTE role and will be based primarily at one practice, with some flexibility to work across a second site just 1.5km away—both located in West Auckland.As a Care Coordinator, you'll be instrumental in supporting individuals and whanau with complex health needs.

You'll work closely with general practice teams and external providers to ensure seamless, culturally responsive care.Key Responsibilities IncludeDeveloping and implementing personalised care plans in collaboration with multidisciplinary teamsSupporting patients with long-term conditions and chronic care managementAdvocating for high-risk families and refugee communitiesNavigating health and social services to ensure holistic supportLeading the IDT function and maintaining strong relationships across the care networkWho We're Looking ForWe welcome expressions of interest from Registered Nurses, Occupational Therapists, and Social Workers, but this particular role is ideally suited to a nurse with experience in chronic care and a strong understanding of the needs of high-risk and refugee populations.You'll Also BringA current APC with no restrictionsDeep understanding of health inequities in Aotearoa and the application of Te Tiriti o Waitangi in primary careCultural competency and confidence working with diverse communitiesPostgraduate studies in health administration or case management (a bonus, but not essential)About ProCareProCare is New Zealand's largest Primary Health Organisation, representing general practices that care for over 850,000 people across Auckland and Northland.
