About the role :
We are looking for a Pou Haapai (Support Worker) to join our team in Taamaki Makaurau. The purpose of the role is to support and navigate across all Early Years and Community Health services. You will be the first point of contact for our whaiora entering and engaging in our service, taking an in‑depth assessment of their needs and wellbeing aspirations, and developing a whānau care plan. This plan provides whānau and the care team with a pathway to wellbeing, supporting referrals, taking immediate action, and addressing the highest priority issues for the whānau.
As a Pou Haapai (Support Worker), you will work with whānau across Taamaki, acting as a navigator, coordinator of care, and advocate for their wellbeing. You must be able to engage at all levels of the system, build trust with whaiora and their whānau, and have an in‑depth knowledge of the local care system, especially services for Early Years. You will work autonomously, booking and scheduling appointments in clinic, external settings, or at the whānau’s home, managing your own caseload of whānau with active care plans. Caseloads may range from 20 to 40 active care plans. The role is full time, Monday to Friday, 8 : 30 am to 5 : 00 pm. Please only apply if you can commit to these days and hours.
