About The Role
The Needs Assessor / Service Coordinator role focuses on undertaking InterRAI Home Care assessments for older people with complex needs, identifying both strengths and areas of support.
Working closely with clients and their whanau, you will coordinate services that enhance quality of life and promote Hauora in the home setting.
Training in the InterRAI system will be provided, but a strong level of digital confidence and adaptability is essential.

About the Team / Service / Location
Support Net is part of the Regional Community team based in Tauranga.
The Health of Older People team works in collaboration with kaumatua, whanau, community providers and other health professionals to ensure safe, effective care plans are in place.

What You Will Bring
Current registration and Annual Practicing Certificate with a relevant New Zealand health professional body.
Experience working with older adults and their whanau in a community setting
Understanding of InterRAI assessment processes (training provided)
Strong digital literacy and ability to quickly learn new systems
A full New Zealand driver's licence and willingness to travel for client visits
