About the Role
The Older Adult Rehabilitation and Allied Health (ORA) Wellington Community Team has an exciting opportunity for an innovative Mātanga Tapuhi Nurse Practitioner (NP) position or a NP Candidacy to registration for this area of practice.
This primary care focused role, based in Newtown, Wellington, improves access to comprehensive gerontology assessment (CGA) and coordinated interventions to support older adults to live safely in their homes. To improve equity, the Mātanga Tapuhi NP will work with Māori and Pacific people aged 55 + years who present to the Community ORA team with diagnoses usually associated with older people that impact their function and social well-being. The Mātanga Tapuhi NP role is integral to providing an individual and population-based approach to improve timeliness to specialist orthopaedic assessment and decision making, improving experience and outcomes for patients and whānau.
One of the key objectives of our wider service is to support Primary Health Care access to specialist gerontology assessment and advice in the community. The Mātanga Tapuhi NP role will contribute to this service objective by providing a flexible and timely collaborative service linking with the patient's primary care provider. The NP will assist with identifying acute or emergent issues and offering treatment guidance and care to prevent further deterioration or loss of function. As an autonomous practitioner you will be working in a team, working closely with our Geriatrician, Nursing and Allied Health Teams.
The salary range for this role is ;Senior Nursing Grade 6 as Nurse Practitioner (NP) with registration as NP as per PSA-Mental-Health-Public-Nursing-RONZ-CA-31-Mar- -Dec-2024 or NZNO-HNZ Collective Agreement or Grade 3 if appointed to NP Candidate role in a NP Candidacy.
About the Team / Service Location
The Wellington ORA team sits within the Community Older Adults, Rehabilitation and Allied Health service and has two other Mātanga Tapuhi NP's. We are a large, friendly, interdisciplinary team offering you access to Physiotherapists, Social Workers, Occupational Therapists, Nurses, Speech Language Therapists, and Dietitians. You will also have support from the Nurse Director and linkage with the Nurse Consultant for Advanced Practice. Given that we are now part of a single District with Hutt Hospital. you will also be linking with Mātanga Tapuhi NPs and NP Candidates working within the Capital Coast and Hutt Valley Community ORA teams.
We enjoy good relationships with Primary Health and a broad range of government and community providers. This collaboration makes it easier for our staff to provide timelier, effective and equity focused health services. You will be involved in the ongoing evolution of these collaborative partnerships as we develop our practice to respond to the diversity of the Wellington District.
Our Multidisciplinary team works well together, sharing knowledge and skills as we provide collaborative assessment and interventions.
