The role is located within the Hutt South Community Mental HealthTeam in Lower Hutt.The Matanga TapuhiNurse Practitioner (NP) - Mental Health is anautonomous practitioner who works independently and collaboratively withmultidisciplinary teams (MDT) and providers to support the safe transition ofadults and older people with complex mental health needs to the residentialcare providers.
Tangatawhaiora from the district and central region transitioningthrough Kelvin House from mental health and hospital speciality servicesrequire comprehensive assessment to enable bestmanagement for significant mental health and often complex physical healthneeds.
The Matanga Tapuhi NP role will support optimising careprovision using an individual and population-based approach to collaborativelysupport healthcare needs for these vulnerable tangata whaiora with a diverse range of mental health conditions.Referrals for this service mayoriginate from rehabilitation, acute, or community pathways where recoveryfocused approaches are no longer appropriate and the individual would benefitfrom long term supportive care in a secure environment and working with theLower Hutt South Community Mental Health Team.
As aNurse Practitioner part of the role will be working closely with the team atthis unit to provide clinical oversight of this cohort of clients.
This caninclude being the MHA Responsible Clinician and providing MH care and supportand will also be working within the Hutt CMHT to provide services for peoplewith mental health need
The team is a multidisciplinaryteam with service provision for people with mental health needs.
We are looking for a recognised advanced practice nurse that has apassion for ongoing learning and the ability to adapt to work effectively withdifferent learning experiences.
