Care Coordination currently have positions available within our Capital and Coast team based in Mana, and Hutt Valley team, based in Lower Hutt.
As a Care Manager no two days are the same, you will undertake assessments in the community using the interRAI suite of tools and your clinical expertise.
Working in partnership with the client, their care giver / family / whanau, you will be developing plans to enable older clients with complex needs to remain safely in the community.
Education, and communication with other providers, is pivotal to this role.

To be successful in this role you will :
Be a registered health professional (including assessment within your scope of practice).
This could be a Registered Nurse or Allied Health Practitioner
Have a high level of computer literacy
Have experience in working with older people, and / or chronic care management, in a multi-disciplinary environment
Be available to work
Experience in clinical / social needs assessment, older person's health, rehabilitation, or primary care is desirable.
A current NZ practising certificate (or equivalent) and drivers licence are essential.
A comprehensive orientation programme is in place to ensure you receive training on the use of interRAI, and other areas that are new to you.
