This position will ensure care pathways mitigate avoidable, unjust or unnecessary barriers caused by the system of health care for Pacific Peoples.In this role you will be accountable for reviewing and advancing the delivery of quality care services with specialty nursing focus on Pacific patients and their families across the planned care pathway.
The ultimate purpose is to improve care services for Pacific patients and their family\'s journey and understand and fulfil strategies that best support Pacific to advance through their planned care pathway journey and receive timely access to their treatments.
The Clinical Nurse Specialist will work collaboratively with the appropriate services to advocate and build effective relationships and networks to ensure culturally responsive care and treatment are delivered and accessed by all Pacific patients.About the Team / Service / LocationThe CNS Planned Care Pacific team sits within the Pacific Health Service at Counties Manukau District.
Our multidisplinary team comprising of nurses, social workers and navigators works in close collaboration with hospital services to support Pacific patients and their families, with a strong focus on improving health outcomes.About youAs a Clinical Nurse Specialist, you\'ll be able to work collaboratively across the sector and organisational boundaries to influence the nursing contribution to better outcomes for our whanau and communities.A current RN Practicing Certificate with the Nursing Council of New ZealandMinimum five years of relevant clinical experience and expertise post registrationPG Dip completed and, commitment to completion of Master of NursingExperience working with patients with complex health and social needsExcellent written and verbal communication skillsMust speak a Pacific island languageAdvanced patient assessment skillsWorking knowledge of Treaty of Waitangi and its application to practiceWorking for Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
