• We are seeking a dedicated Clinical Needs Assessor to provide screening, triaging, and coordination of appropriate services based on comprehensive assessments.

In this role, you will identify the need for specialist assessments, therapeutic interventions, and ongoing support for individuals aged 65 and over, as well as those aged

This role will require occasional travel throughout Northland to complete home assessments.to view the position descrption.
