• About the Role
• We are seeking an enthusiastic, passionate and skilled registered health professional to join our Health of Older People team at Disability Support Link.

This will be a varied and exciting role that will work alongside clients, families, local communities and inpatient settings so that the process of a needs assessment and service coordination reflects both community strengths and client needs.You will be working in partnership with older people who have disability needs, and younger people with long-term health conditions, to achieve optimum health and well-being through appropriate assessment, planning, intervention, evaluation and education.
