About the role
We offer treatment and support services to a wide range of consumers including Tangata Whaiora.
Currently struggling with severe and enduring mental health conditions, who have experienced difficulty in maintaining their wellness with previous community mental health services.
About You
We are looking for an enthusiastic Registered Nurse to join the North Community team as a Case Manager who will work closely with consumers and their whanau.
Under the North Community team, the case management position entails providing comprehensive psychiatric assessment, case management, and therapeutic interventions for our clients and their family / whanau.
You will work as a multi-disciplinary team member, where there is a strong focus on returning clients to primary health care sector following specialist input.
You will be experienced with managing and responding to crises.
You will have experience working in mental health.
Community experience is preferred.
You must who have registration with a current APC and scope to practice in mental health.
Full Driver - s license is essential.
