District Nurse / Community Nursing Sister

The closing date is 17 September 2025.

A rare opportunity has arisen for a District Nurse or Community Nursing Sister to lead the Stour District Nursing Team, within the Christchurch Locality.

The team has a breadth of experience and a shared goal to offer the highest standard of care to our patients.

We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in their own home.

Your role will be to lead the team and case manage the provision of care within an individual\'s home or residential home, including palliative care and long term conditions management.

You will work autonomously using your own initiative to deliver holistic care to adults with physical health needs.

You should possess excellent leadership abilities, interpersonal skills and time management skills.

You should have experience in caseload management and be able to prioritise work.

The role is diverse, challenging and extremely rewarding.

The role is full time and will include working weekends and bank holidays on a rota basis.

Ideally candidates will have the Specialist Practitioner District Nurse qualification or a willingness to undertake in the future.

Secondment opportunities could support future applications for this course.

Main duties of the job

Provide a high quality person centred approach to care delivery which always considers people\'s safety, privacy and dignity.

Provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.

Provide specialist advice to others regarding the management and care of patients / service users.

Devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

Plan, implement and review health improvement programmes in a range of settings.

Provide supervision, mentorship, training and clinical support to students and trained practitioners.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Person Specification

KNOWLEDGE, SKILLS AND TRAINING

Registered practitioner to degree / diploma level supplemented by post registration diploma level specialist training and / or short courses or demonstrable extensive experience in the relevant specialty

Membership of the relevant Professional Body

Practice Assessors\' course / experience

JOB SPECIFIC EXPERIENCE

Experience at Practitioner Band 5 level

Experience of specialist working

Experience of developing specialist programmes of care for an individual or groups of patients / clients and of providing highly specialist advice

MANAGERIAL / SUPERVISORY EXPERIENCE

Experience of providing clinical supervision and mentoring to junior staff

Experience of devising and delivering training

INFORMATION TECHNOLOGY / RESOURCES

Experience of using electronic patient / service user record systems

Able to analyse data and produce reports using Microsoft Excel and Word

Disclosure and Barring Service Check
