About the role and what you'll do

The Care Coordinator is a key connector within the primary health team — helping patients and whanau navigate their health journeys with confidence, support, and continuity of care.

Coordinate and facilitate care across general practice, community, and secondary services, ensuring smooth transitions and consistent follow-up.

Work with patients and whanau to identify needs, remove barriers, and enable access to the right care at the right time.

Collaborate with GPs, nurses, and the wider Ora Toa Team to provide proactive, wrap-around support for those with complex or ongoing needs.

Support health literacy, self-management, and strong continuity of care through trusted, ongoing relationships.

Deliver holistic nursing care focused on patient and clinic needs, aligned with Ministry of Health and local health targets, including immunisation, cervical screening, smoking cessation, sexual health, heart and diabetes checks, and mental health referrals.

Participate in clinical audits and quality improvement programmes to ensure continuous service excellence.

Use clinical and client management systems to plan, record, and track care coordination activities, ensuring information is shared appropriately and care remains connected.

Uphold safety standards and proactively identify and manage risks to ensure a safe environment for patients and colleagues.

Support and mentor new graduates and students, fostering professional growth and teamwork.

Promote the use of Te Reo Maori and tikanga Maori within our services, embracing our cultural values.

Contribute to environmental sustainability initiatives and ensure timely, accurate financial billing processes.

What you'll need to be successful in the role

A Registered Nurse with a current Nursing Council of New Zealand practising certificate.

Bachelor's degree or equivalent experience (postgraduate qualifications preferred).

Commitment to ongoing professional development and adherence to clinical best practice guidelines.

Proficient or willingness to develop skills in Te Reo Maori and tikanga Maori.

Full and clean driver's licence.

Strong communication skills, teamwork mindset, and ability to take initiative and innovate.

Vaccination authorisation and cervical screening qualifications (preferred).

Experience using Medtech, Noted and office technologies (Office 365).

CPR Certificate (minimum Level 4) and current indemnity insurance.
