About The Role
We're seeking an experienced and motivated Registered Nurse (RN) to be part of and support the development of our Hospital in the Home (HITH) service at Hauora a Toi, Bay of Plenty.
This is an exciting opportunity to build and shape a new service and team delivering acute, hospital-level care in the home environment.
The HITH model supports admission avoidance, early discharge, and improved patient experience by providing timely, patient-centred care in partnership with patients, whanau, and community teams.
Key Responsibilities Include
Participating in the daily operations of the HITH service
Engaging and participating as a new multidisciplinary team
Managing resources to ensure safe and efficient care delivery
Driving quality, safety, and continuous improvement initiatives
Supporting clinical decision-making
Building strong partnerships across inpatient, community, and primary care teams
Contributing to ongoing service design and future growth
About the Team / Service / Location
The Hospital in the Home (HITH) service provides acute clinical care for patients in their own homes, enabling safe alternatives to hospital admission and supporting early discharge from inpatient settings.
You'll belong to a committed team to deliver responsive, high-quality care that aligns with Te Whatu Ora's commitment to equitable, patient-centred outcomes.
About You
You're a confident and adaptable nurse who thrives on coordination, problem-solving, and supporting others.
You're comfortable leading teams through change and developing new services from the ground up, with a strong focus on quality, safety, and innovation.
You'll Bring
Current registration with the Nursing Council of New Zealand (APC required)
Experience in acute, community, or ambulatory care
Proven ability to support, and develop a new or growing team
Sound understanding of quality improvement,
Excellent interpersonal and communication skills
Commitment to partnership with Maori, equity, and continuous learning
A postgraduate qualification (or working towards one) in management, leadership, or a relevant clinical area (desirable)
