The HITH model supports admission avoidance, early discharge, and improved patient experience by providing timely, patient-centred care in partnership with patients, whanau, and community teams.As the RN – HITH, you'll provide operational support across the service.
You will be part of the team that develops and participates in this new model of care — being part of the team, establishing safe care practices and processes, and embedding high clinical standards from the outset.Key responsibilities include : Participating in the daily operations of the HITH serviceEngaging and participating as a new multidisciplinary teamManaging resources to ensure safe and efficient care deliveryDriving quality, safety, and continuous improvement initiativesBuilding strong partnerships across inpatient, community, and primary care teamsContributing to ongoing service design and future growthAbout the Team / Service / LocationThe Hospital in the Home (HITH) service provides acute clinical care for patients in their own homes, enabling safe alternatives to hospital admission and supporting early discharge from inpatient settings.You'll belong to a committed team to deliver responsive, high-quality care that aligns with Te Whatu Ora's commitment to equitable, patient-centred outcomes.About youYou're a confident and adaptable nurse who thrives on coordination, problem-solving, and supporting others.
