Our nurses play a vital role in supporting patients transitioning from hospital to home, and in working alongside GP practices to provide care in the community.Delivering home-based interventions including : Intravenous therapyPalliative careWound careCompression bandaging and Doppler assessmentsMedication managementStoma and incontinence careRespiratory support and home oxygen therapyAssessing patients for their personal care needs and developing care plans.
