We are looking for a registered Social Worker with a passion for working with, and supporting tamariki and their whanau within a health care lens, to join our Community Child Health Team based at Waitakere Hospital.You will be joining an experienced and highly skilled multi-disciplinary paediatric community team who are committed to innovative best practice in the context of tamariki-focussed / whanau centred care, that is based on the strengths and goals of the whanau.

We are seeking applications from like-minded clinicians who share our passion.Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.About the RoleAs a Social Worker in the Community Child Health Team, you will be working in a multi-disciplinary environment, providing social work support for the whanau of tamariki who are impacted by acute and / or long term health care needs.

The role is primarily based in the community providing support for whanau in their home, with the addition of some in-reach social work support to the Rangatira Paediatric Ward.Following an orientation and settling in period, it is expected that you will participate in the Child, Women and Family On-Call roster providing support across our inpatient wards for child health, womens health and maternity wards.

The on-call roster is active on weekends and public holidays from 8am-4.30pm.There may also be times where our Child Health Social Workers work across other Child, Women and Family areas, outside of the on-call roster, to offer support and service delivery across our division as needed - so a level of flexibility and adaptability would be required.

You will be provided regular supervision with the support of our Social Work Clinical Leader who works across both North and West Community Child Health Teams, as well as the Professional Clinical Leader for Social Work.The role is based primarily at Waitakere Hospital, but travel to other Waitemata sites (North Shore and West Auckland) will be required in this role.About the Team / Service / LocationOur Community Child Health Team is made up of dedicated allied health and nursing clinicians who share a passion for supporting tamariki and their whanau who are faced with various health and disability challenges to live their best lives at home.

Working in collaboration and in partnership with our whanau is central to our work, as is the drive to be strengths based.

We have a hard-working team, that are closely connected - supporting each other on a daily basis, but who also enjoy bringing a sense of fun to their work environment.

We maintain close working relationships across our inpatient paediatric areas and our community based Child Health and Womens Health services.About youOur ideal person will be passionate about providing care and improving health and social outcomes for tamariki and their whanau.

You will have proven ability to work autonomously as well as collaboratively in a supportive multi-disciplinary team environment.For this role you will need to have : A current APC and be registered with the Social Workers Registration Board (SWRB)Child Protection knowledge and experienceAwareness of the health, disability and social services sectorBe familiar with the developmental and / or health needs of tamariki / rangatahiAwareness of the support required by Whanau who have tamariki / rangatahi with a disability or a health needHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
