About the Position | Te Mahi
We are expanding our Homecare Clinical Team and are looking for an Enrolled Nurse or New Graduate Nurse to join our Wellington team.
This is a permanent part-time role (32 hours per week, Monday–Sunday rostered).
The position involves visiting patients in their homes, applying nursing knowledge and assessment skills, case management, and patient education.
You will develop positive working relationships with GP practices and other referrers.
You will be empowered to provide continuity of care for our patients from beginning to end, with a comprehensive orientation, ongoing clinical support, and access to continuing education.
The role is highly autonomous and involves visiting patients in their homes to apply practical nursing knowledge and assessment skills, case management, and patient education.
It also involves developing positive working relationships with GP practices and other referrers, and provides continuity of care with strong orientation and ongoing clinical support.
In this role
You will use your skills to assess and plan Homecare support for our Service Users.
You will be hands on with service user assessments and Community Support Worker training and competencies.
You will follow up and review service user goals and progress on a regular basis.
You will enable service users to live their best lives.
You will ensure accidents and incidents are reported in a timely manner and escalate service user concerns to the Clinical Lead.
You will work in collaboration with Clients Service Managers and the Homecare Clinical Lead to provide outstanding services to our service users in the community.
You will be competent in your field with the ability to learn different skills and gain broad knowledge around medication management, enteral feeding, falls and pressure injury prevention, and wound care.
You will work holistically using a client-centered approach with strong local knowledge of the health system.
You will have the ability to communicate with primary and secondary healthcare and an understanding of ACC and MOH community contracts is an advantage.

Experience working in the community is desirable
Current registration and NZNC practising certificate
The ability to work autonomously with a strong solution focus
Strong MS Office skills
Attention to detail
Strong and positive communication skills
Ability to work in a complex and changing environment
Critical thinking
Clean, full New Zealand Driver's license
Must have the legal right to work in NZ
We offer
Competitive Pay Rates depending on years of practical experience (clinical workers can range from $70,000 to $85,000 annually)
