As a Relief Counselor, you will support individuals facing mental health challenges by helping them live independently and beyond their perceived limitations.Adult Community Clinical Services is a Department of Mental Health funded service offering evidence-based interventions within a clinical model.
The individuals served are diagnosed with mental illness or dual diagnosis and live in their own homes, apartments, or group settings.
Our goal is to assist clients in transitioning to independent living, building skills, and achieving personal goals through a supportive, multidisciplinary team of clinicians, counselors, nurses, peer staff, and others.Relief Counselors contribute to our mission by providing direct support, ensuring daily needs are met, and maintaining a safe, inspiring environment.Other Key Responsibilities : Develop and implement individualized, person-centered treatment plans.Assist individuals with accessing transportation.Create opportunities for community engagement and relationship building.Complete all required documentation accurately and timely.Note : Some positions may qualify for an additional hourly differential based on skills, location, training, and population served.
