About The Role

• Provide direct patient care, focusing on individuals and whānau at greatest risk of poor health outcomes.
• Actively support hospital avoidance through medication management and optimisation.
• Collaborate with the interdisciplinary team (IDT) to develop and implement strategies that address the specific needs of our population.
• Conduct medication reviews, reconciliations, and assessments to ensure safe and effective use of medications.
• Educate and empower patients and whānau on their medication regimens, including potential side effects and adherence strategies.
• Manage long‑term conditions through medication initiation and continuation (prescriber role only).
• Lead medication utilisation evaluations and audits to continuously improve care delivery.
• Contribute to the development and management of medication‑related policies and procedures.
