About The Role
- Provide direct patient care, focusing on individuals and whānau at greatest risk of poor health outcomes.
- Actively support hospital avoidance through medication management and optimisation.
- Collaborate with the interdisciplinary team (IDT) to develop and implement strategies that address the specific needs of our population.
- Conduct medication reviews, reconciliations, and assessments to ensure safe and effective use of medications.
- Educate and empower patients and whānau on their medication regimens, including potential side effects and adherence strategies.
- Manage long‑term conditions through medication initiation and continuation (prescriber role only).
- Lead medication utilisation evaluations and audits to continuously improve care delivery.
- Contribute to the development and management of medication‑related policies and procedures.
We Are Looking For Someone With
A Bachelors of Pharmacy or equivalent qualification.Registration with the Pharmacy Council of New Zealand.Experience in a primary healthcare setting or a strong desire to transition.A passion for whānau‑centred care and a commitment to health equity.Excellent communication and interpersonal skills to build strong relationships with patients, whānau, and healthcare professionals.A collaborative and team‑oriented approach.Strong analytical and problem‑solving skills.This Role Offers
