About The Role

Provide direct patient care, focusing on individuals and whanau at greatest risk of poor health outcomes.

Actively support hospital avoidance through medication management and optimisation.

Collaborate with the interdisciplinary team (IDT) to develop and implement strategies that address the specific needs of our population.

Conduct medication reviews, reconciliations, and assessments to ensure safe and effective use of medications.

Educate and empower patients and whanau on their medication regimens, including potential side effects and adherence strategies.

Manage long-term conditions through medication initiation and continuation (prescriber role only).

Lead medication utilisation evaluations and audits to continuously improve care delivery.

Contribute to the development and management of medication-related policies and procedures.

We Are Looking For Someone With

A Bachelors of Pharmacy or equivalent qualification.

Registration with the Pharmacy Council of New Zealand.

Experience in a primary healthcare setting or a strong desire to transition.

A passion for whanau-centred care and a commitment to health equity.

Excellent communication and interpersonal skills to build strong relationships with patients, whanau, and healthcare professionals.

A collaborative and team-oriented approach.

Strong analytical and problem-solving skills.
