About the Initiative
ProCare's Comprehensive Primary Care Team (CPCT) is a pioneering workforce model designed to improve health equity by embedding allied health, social work, and pharmacy professionals into general practice teams. Fully funded through June 2026, CPCT enhances access to care for at-risk populations while supporting general practice teams with integrated, whānau-focused services. This initiative enables free patient access to essential services, supports workforce development, and fosters innovative, community-based care.
About the Role
The primary purpose of the role is to provide front-line physiotherapy assessment and care interventions as an integrated member of the general practice inter-disciplinary team. Working closely with the GP's, nurses and the wider team, the CPCT Practice Physiotherapist's duties will include :
- Assessment, diagnosis, and treatment of acute and chronic conditions within scope of practice
- Provision of individualised exercise or rehabilitation programmes, including manual therapy, self-management support and education for areas of health, including but not limited to musculoskeletal and other long‑term conditions.
- Prioritisation of people and whānau to support hospital avoidance, early supported discharge from hospital and those at greatest risk of poor health outcomes.
- Clear strategies to simplify care for whānau where able, such as delegation to non‑regulated roles where appropriate and promotion of self‑management approaches and intensifying interventions for those with the most complex needs.
Qualifications and Experience
We are interested in hearing from Physiotherapists with a range of experience levels and background at this stage, however our successful applicants will typically need to have :
Bachelor of Physiotherapy or equivalent overseas qualification; Qualified physiotherapist with current APC with no restrictionsRegistration with Physiotherapy Board New ZealandPrimary or community healthcare experienceComprehensive understanding of inequities in access and health outcomes in Aotearoa, New Zealand and detailed knowledge of Te Tiriti o Waitangi and its application to shaping all efforts within primary careStrong cultural competency and confidence with sufficient knowledge of the peoples, languages and cultures that form our most at-risk communitiesExcellent critical appraisal skills and being able to identify the best evidence-informed solutions to clinical and practice questions and issues.About ProCare
