About the Team / Service / LocationWe - re seeking Casual District Nurses to join our supportive nursing team, working out of the Community Health Service, Ewart Building, Coromandel Street, Newtown.District Nursing is a dynamic and evolving specialty.Our nurses play a vital role in supporting patients transitioning from hospital to home, and in working alongside GP practices to provide care in the community.Delivering home-based interventions including :
Intravenous therapyPalliative careWound careCompression bandaging and Doppler assessmentsMedication managementStoma and incontinence careRespiratory support and home oxygen therapyAssessing patients for their personal care needs and developing care plans.Direction and delegation of the Health Care Assistants (HCAs) and collaborating the wider Community Health team to ensure safe, coordinated care.
