Purpose of Position :

To support clinical quality improvements and initiatives across the CHT Care Homes.

Key Accountabilities :

• Work collaboratively with the Clinical Quality Lead to achieve and deliver the Organisational Plan
• Provide support to the Clinical Quality Lead to mitigate clinical risk across the CHT care homes
• Collate data, review, analyse and report
• Identify themes and trends and recommend / develop initiatives or training
• Provide education and training to support quality initiatives and improvements
• Participate in continuous improvement projects across the care homes
• Maintain competency in knowledge and clinical practice appropriate to the role
• Support CHT in creating and maintaining a responsive and respectful culture that integrates our values, enables diversity and is culturally appropriate.
