About the Role
Supportlinks is the Needs Assessment and Service Coordination (NASC) service which provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district for people with acute / short term needs, people who have long term health conditions, and older people / kaumātua with age related health conditions.Supportlinks work primarily with a community focus however also has a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged.Supportlinks currently has a temporary 0.8 FTE position covering parental leave for up to one year. The position can be
based in either our Palmerston North or Levin office
. We are looking for a person who is enthusiastic, energetic, resilient, and has a “can do” attitude, to join our supportive team and would welcome applications from candidates who hold an annual practicing certificate in any of the following disciplines; Nursing, Occupational Therapy, Physiotherapy or Social Work.

About you
an ability to work within a changing health environment

strong organisational and time management skills
skills to use a laptop and learn new assessment software
a full, current, driver's licence
