Contract : 6 month Fixed Term, Full Time

About the Role

We have a rareopportunity for a 6 month contract for a care manager (social worker,occupational therapist, registered nurse) to cover a secondment. You will joina vibrant team delivering high quality mental health care to a diversepopulation.

Covering the Southernand Eastern suburbs of wellington affords us a diverse caseload of clients withequally diverse needs. About you

• Able to work autonomously as well ascontribute to the MDT.
• Creative,innovative thinking and problem solving.
• Applicationof best practice models of care.
• Awillingness to learn and develop your practice.
• Excellentcommunication and interpersonal skills.
• Self-motivatedand skilled in developing relationships with a variety of other professionals.
• Registered with the relevant board in New Zealand.
• Youwill have experience providing comprehensive assessments and risk managementplans, formulating and articulating treatment needs and plans, and be able tocoordinate and prioritise treatment needs.
