About The Role
The Needs Assessor / Service Coordinator role focuses on undertaking InterRAI Home Care assessments for older people with complex needs, identifying both strengths and areas of support. Working closely with clients and their whānau, you will coordinate services that enhance quality of life and promote Hauora in the home setting. Training in the InterRAI system will be provided, but a strong level of digital confidence and adaptability is essential. This is a rewarding opportunity to work within a team that supports equity, compassion, and excellence in care.
About the Team / Service / Location
Support Net is part of the Regional Community team based in Tauranga. The Health of Older People team works in collaboration with kaumātua, whānau, community providers and other health professionals to ensure safe, effective care plans are in place.
