Cranford Hospice Trust (CHT) provides the best palliative care, education, support, and advice to the people of Hawke's Bay. Our vision is to help people make the most of living and dying within a compassionate community. We are committed to helping people make the most of the time they have left and enabling them to live every moment—whether they are at home, in an aged care facility, or have come to us for a short stay in our inpatient facility.
Our Pharmacist works as part of the interdisciplinary team to improve the quality of life of specialist palliative care patients through the appropriate use of medication for the best possible health outcome and comfort of the patient. The pharmacist has responsibility and oversight for all facets of pharmacy service provision within Cranford Hospice and coordinates and supports the team to meet all relevant standards, regulatory, and legislative requirements.
Key Responsibilities
- Coordinate and provide specialist palliative care pharmacy practice as part of the interdisciplinary team.
- Manage medicines procurement, storage, and disposal effectively and sustainably.
- Ensure compliance with all relevant standards, regulations, and legislation requirements related to pharmacy services.
- Engage in building the capacity of the clinical team in relation to medicines management in Specialist Palliative care.
- Conduct medicines reconciliation at admission and discharge.
- Educate patients, families, and whānau on medications in partnership with medical and nursing staff.
- Participate in interdisciplinary team meetings and contribute to the development of care plans.
- Liaise with General Practice Teams, community pharmacies, Residential Aged Care facilities, and families regarding medications for patients on discharge.

FTE : 16–20 hours per week (around 0.5 FTE)—days and hours worked can be negotiated
Salary banding : $100,000–$104,000 per annum pro rata
