We are passionate about helping whanau to become smokefree, and we\'re on the lookout for someone to join our team on a permanent full-time basis.We are changing people\'s lives through empowerment, equipping them with skills and resources and supporting them in their journey to becoming smoke-free.
The key goal of our RSQ Kaimahi - Practitioner, is to provide personalised support including health education.Some of your day-to-day responsibilities will include assessing the needs of clients to help them create a support plan, utilising face-to-face motivational interviewing skills, scheduling visits to maximise contacts, and administration activities.The ability to self-manage, and enable behavioural changes in others will be essential, alongside being able to monitor progress and assist with the facilitation of groups and health promotion activities.
This is an incredibly rewarding role with an opportunity to make a tangible impact on the lives of whanau across Tamaki Makaurau.As the Ready Steady Quit Kaimahi - Practitioner, some of the key aspects of this role will include : Be part of the wider healthcare team to support, coach, and walk alongside whanau as they navigate though their health and wellbeing journey.Promote self-management and achievement of goals, help to navigate and connect with any necessary partner services.Reduce barriers to accessing support by providing services where health conditions (including mental distress and addictions) financial and social challenges, housing, training and employment can all be addressed in a primary care setting through a multidisciplinary support team.Travelling and working closely across multiple general practice teams, participating as a regular team member to help build strong working relationships and rapport.Key Skills / Attributes RequiredExperience working in health care directly with patients, community groups and organisations.Ability to understand, communicate and effectively interact with people across all cultures.Experience especially in working with Maori & Pacific Island population is preferred, ability to build rapport and strong professional relationships.Completed or are willing to complete the Stop Smoking Practitioners Training Programme.A desire to understand and appreciate Te Tiriti o Waitangi and its application to ProCare and how it shapes our efforts.Flexibility to travel across multiple clinics within your delegated regions.Plus, demonstrated ability to work independently as well as within a team, and proficiency in IT and software systems.Pre-requisitesAs this role works directly with people in the community, applicants must : Be a non-smokerHave no prior criminal convictions (excluding exemptions under the Criminal Records (Clean Slate) Act
