Key focus for the role is : Working together to seamlessly transition patients out of acute care areas and onwards to their discharge destinationWorking in close collaboration with clinical areas and the Integrated Operations Centre to prioritise movements that achieve positive hospital wide system flowExploring proactive initiatives to support the discharge processEnsuring patient and whanau discharge needs are met, including documentation, transportation, education, while bringing hospitality to the conclusion of their hospital episodeAbout youA high level of professionalism and communicationAbility to work well within a multi-disciplinary teamAbility to work in a flexible mannerGood IT and organisational skillsProactive and motivatedAbout the Team / ServiceEmploying over 2500 people, MidCentral offers a supportive work environment that accepts and values all employees.
