About the Position :
As a Health Coach, you will work collaboratively with whānau whaiora and the THINK Hauora team to promote self-management skills and provide support tailored to the needs and aspirations of whānau. You will liaise with various mental health and addiction services alongside primary health care teams, offer holistic monitoring of wellbeing needs, and coordinate access to the best support options available.
Supporting the health and wellbeing of New Zealanders is changing and evolving for the better. Emphasis is placed on reducing barriers to accessing services and embedding a 'skills not pills' approach within primary care that empowers people to lead their own physical, mental, social, spiritual wellbeing.
THINK Hauora is keen to hear from applicants interested in a career path as a Health Coach within primary care. This role offers you a chance to positively impact the lifestyle and wellbeing of a range of people enrolled in general practice settings. You will be working in partnership with patients and whānau, GPs and nurses to help patients gain the knowledge, skills and self-confidence to reach their health goals. You will be empowering people with their lifestyle changes (smoking cessation, exercise), promoting self-management (especially for managing long-term conditions like diabetes) and helping navigate primary care and the wider social services they need to help them succeed.

Your friendly and approachable manner will see you quickly build rapport, and you will be resilient to tackle barriers and obstacles.
