Case Manager

Overview

As a Case Manager at Auckland South Corrections Facility (ASCF), you’ll provide end-to-end case management to support safer communities. You’ll assess rehabilitation and reintegration needs, develop and monitor individual management plans, and work directly with the men in our care to motivate meaningful, lasting change.

This is a full-time position, reporting to the Principal Case Manager, and based in Wiri, Auckland.

Responsibilities

• Assessing prisoner needs using specialist tools like SDAC-21
• Developing and reviewing Prisoner Management Plans in line with best-practice standards
• Delivering brief interventions and motivating positive behavioural change
• Chairing case conferences and coordinating rehabilitation and reintegration activities
• Preparing Parole Board reports and attending hearings where required
• Collaborating with Reintegration Officers, Probation Officers, and community partners
