Brackenridge is inviting Expressions of Interest for a compassionate and experienced In-Home Support Occupational Therapist, Physiotherapist, or Registered Nurse to join our dedicated team. In this role, you will lead the development and implementation of personalised care plans for individuals living with significant impairments or disabilities. You will coordinate and oversee tailored support services that are client-focused, culturally responsive, and compliant with contractual and quality standards. We have over 400 staff in a variety of roles that provide support services for people with a disability, across a wide age range.

What You’ll Do

• Develop tailored support, risk plans and goals based on individual needs and personal goals.
• Providing service coordination by doing all digital rostering and scheduling.
• Monitor client progress and outcomes whilst liaising with clients, their families, and the multidisciplinary team to identify personalised goals and assist in developing rehabilitation plans.
• Oversee Support Workers to ensure seamless service delivery, including remote management.
• Conduct client meet-and-greet sessions to build rapport and understand personal goals.
• Liaise with clients, their families, and the multidisciplinary team to identify personalised goals and assist in developing rehabilitation plans.
