About the Role

Our client is seeking a Nurse Practitioner or Diabetes Clinical Nurse Specialist to join their passionate multidisciplinary primary care team, delivering culturally responsive services across the South Auckland region.

This role is ideal for a dedicated CNS looking to make a measurable impact in the community while enjoying strong team support, professional autonomy, and the chance to work with whānau in meaningful ways.

Key Details

Location : Community-based across South Auckland

Hours : Full-time or part-time - choose a schedule that works for you

Remuneration : Competitive salary aligned with experience + benefits

Key Responsibilities

• Administer vaccinations in the community, including outreach settings
• Work collaboratively with an MDT to manage complex health presentations
• Deliver expert diabetes care including medication titration, phlebotomy, and treatment planning
• Create and implement personalised, culturally appropriate care plans for clients and their whānau
• Participate in the Diabetes South Auckland Pilot Programme, supporting equitable health outcomes for Pacific communities

Requirements

Candidates MUST hold :

• A current APC with the NZNC
• A Diabetes CNS qualification or nurse practitioner qualification

Preferred candidates will exhibit :

• Proficiency in phlebotomy and community-based clinical work
• Experience in diabetes management, including insulin and medication titration
• Confidence in vaccinations, ideally with experience in chronic care and outreach services
• NZ experience and a high level of cultural competence, especially when working with Pacific communities
