About the Role
Your day will primarily involve contacting and visiting older people in their own home environments to determine their ability to maintain their independence. This includes working with clients their whanau and the wider community support service agencies to ensure the client's care needs are met.
About the Team / Service / Location
You will work as part of our multi-disciplinary rehabilitation team to provide clinical assessment for older people with long-term conditions requiring support services to maintain their independence in the community.
