- The Home team is available to provide hospital avoidance and supported discharge of patients working across a range of settings including the community, Emergency Department, and in-patient areas.
The Home team are a comprehensive, integrated service that are responsive to patient and service needs.
The Home team are focused on delivering one car up the driveway, one assessment and one plan of care.This is a fantastic opportunity for a registered nurse interested in working as part of an inter-professional team.
Within this position, you will support the early discharge of appropriate patients from Southland Hospital.
You will monitor their recovery and ensure everyone is able to maintain their independence in their own home.To add to the excitement of this role, as a registered nurse within the Home team, we are reviewing processes to enable the provision of a responsive community-based service for patients, who (while being medically stable) require urgent assessment and immediate coordination of services to prevent Emergency Department presentations or hospital admission.This role covers a Monday to Sunday service between the hours of 8am to 6pm, and also includes public holiday work.

About youThis role requires a nurse who is adaptive to change, resourceful, flexible to service needs and future development and a genuine passion for working in the community.Within this position, you will require :comprehensive clinical assessment and problem-solving skillsthe ability to work autonomously and collaboratively within the inter-professional teamthe ability to direct and delegate to Rehabilitation Assistantsexcellent communication skills to liaise with health professionals, both within the in-patient services and in the community settingrecent experience of working with older adults in acute, rehabilitation, or community settingsYou will need to be a NZ Registered Health Professional, with :a current APCa minimum of two years post registration experience in New Zealanda full clean New Zealand driver's licenceTo be successful in the role, you will have a strong commitment to your ongoing professional development, and it is expected that you will have or be committed to working towards a post-graduate pathway.Competence in assessment of complex patient needs is desired.
