Job SummaryThis role supports healthcare teams by assisting with both clinical and non-clinical patient services under the direction of healthcare providers.Transitions Clinic MissionWe serve the community by providing post-discharge acute care to patients who were in the ED or inpatient at the hospital and do not have a Primary Care Provider.

We strongly encourage diverse candidates, bilingual in Spanish and English preferred, and underrepresented groups to apply.Key ResponsibilitiesTaking vital signs, documenting patient information, scheduling appointments, and managing patient flow.Interacting directly with patients to ensure their comfort and address concerns.Collaborating with physicians, nurse practitioners, and nursing staff to facilitate efficient care.Essential FunctionsInterview patients to obtain medical information and measure vital signs, weight, and height.Show patients to examination rooms and prepare necessary equipment.Record medical history, vital statistics, and test results in medical records.Perform general office duties such as answering phones, taking dictation, and completing insurance forms.

Act as a liaison with other departments and advocate for patients with a positive customer service approach.Collect laboratory specimens, log them, and prepare for testing.Prepare treatment rooms, keep them clean and stocked.Clean and sterilize instruments and dispose of contaminated supplies.Perform other duties as assigned and comply with all policies and standards.Knowledge, Skills, and AbilitiesStrong interpersonal and communication skills.Ability to prioritize tasks in a busy environment.Accuracy and attention to detail.Knowledge of privacy and confidentiality rules (local, state, federal).

Ability to take vital signs, manual blood pressures, and collect blood samples.Knowledge of medical terminology, procedures, and protocols.Proficiency in EHR systems and medical office software.Logical reasoning to analyze solutions and approaches.Time management skills.Education and CertificationsHigh School Diploma or Equivalent required.Experience in lieu of a degree is not accepted.Preferred certifications : BLS, Certified Medical Assistant, EMT-B (Massachusetts).
