Position Purpose – Ko te aronga o te turaka

WellSouth's most important relationships are with general practices — the foundation of primary care delivery.

The Practice Partner serves as the strategic and operational connector between WellSouth and its network of general practices, ensuring practices are well supported to deliver sustainable, equitable, and high-quality care.

The Practice Partner delivers responsive, high-quality, and locally tailored support to general practices across the Southern region.

The role ensures that practices are well-positioned to implement new models of care, meet national targets, and achieve our equitable outcomes.

Acting as the WellSouth lead for practices on the basis of partnership.

Partnering with practices in the effective delivery of models of care, system initiatives, and clinical programmes that support practices' clinical and financial viability.

Supporting effective integration between general practice and WellSouth teams.

Championing practice insights to inform WellSouth decision-making.

Contributing to resolving operational barriers affecting practice performance and outcomes.

You will be working collaboratively with our Integration Director and Senior Leadership Team.

Act as the WellSouth lead for practices on the basis of partnership.

Partner with practices in the effective delivery of models of care, system initiatives, and clinical programmes that support practices' clinical and financial viability.

Support effective integration between general practice and WellSouth teams.

Champion practice insights to inform WellSouth decision-making.

Contribute to resolving operational barriers affecting practice performance and outcomes.
