About the Role
The primary focus of the National Advance Care Planning Lead is to lead the national Tō tātou Reo | Advance care planning and clinical communications programme. This involves coordinating and embedding the implementation and use of advance care planning, shared goals of care and the serious illness conversation guide (SICG) across the health sector. This will include planning and designing the programme, monitoring its progress and resolving issues that arise whilst developing and monitoring the vision, purpose and strategy and ensuring training and resources are accessible to a range of audiences.
About you
You will hold a relevant graduate qualification alongside a strong background in strategic and leadership skills paired with in-depth knowledge of Advance Care Planning. You will also have the ability to develop positive working relationships and build detailed networks with stakeholders through your excellent communication skills.
