You will liaise closely with individuals, families and other health professionals, and develop strong links with community services as required to provide integrated care for patient and whanau.
You will have regular supervision and support with continuing professional development.Please note that the position description is available upon request.About the Team / Service / LocationHealth New Zealand | Te Whatu Ora-Taranaki is a team of teams and employs approximately 350 allied health professionals across its services.
You will be joining the physical health social work service; a friendly team of 12 that is diverse in skills and experience, a team that welcomes collaboration, and that values the contribution of each team member.
Physical health social workers practice as part of multi-disciplinary teams in a range of medical, surgical, and rehabilitation settings to support people facing health challenges across the lifespan.About youA successful applicant will have demonstrated their ability to be a clear communicator, a builder of connections with clients and colleagues, and within the wider community, and have excellent time management skills.
You will be independently managing a caseload of varying complexity and completing assessments with an interprofessional lens.
You will be working closely with other health professions.
Requirements include : New Zealand registration with the Social Workers Registration Board and a current Annual Practicing CertificateThe ability to work collaboratively with others to provide integrated care to clients with a diverse range of challenges.Ideally, you'll have two or more years of experience working in a health or disability field of practice, but we are open to applications from an early-career social worker.Working visa / NZ residence (as appropriate).
