The Role
We are looking for a dynamic Primary Health Network Team Lead to oversee the daily operations of our multidisciplinary team.
In this pivotal role, you will lead the delivery of proactive, evidence-based care for people with Long Term Conditions, while supporting General Practices across the region.
You will act as a key connector between practices, THINK Hauora, and other healthcare providers – ensuring seamless, coordinated care.
This role also provides expert guidance on clinical, contractual, and funding matters, and involves regular local travel from either our Palmerston North or Horowhenua office.
You will be responsible for :
Leading a multidisciplinary team to deliver coordinated, high-quality care for patients with long-term conditions
Acting as a key support and liaison for general practices, providing clinical guidance and connecting them with THINK Hauora services
Strengthening partnerships with general practices by coordinating patient care, facilitating referrals, and ensuring smooth communication and continuity across services
Providing financial and contractual support to General Practices to ensure compliance and optimise funding
Leading and supporting the team to deliver high-quality, compliant care through evaluating care plans, monitoring performance, conducting audits, and promoting continuous learning
You will have :
Bachelor's degree in nursing, with a current practicing certificate.
A Prescribing qualification is preferred.
Minimum 10 Years clinical nursing experience in Primary Health, with Practice Management experience
Proven ability to lead and manage a multidisciplinary team.
Strong leadership skills with experience motivating teams and providing clinical guidance
