The role combines direct clinical care, expertsymptom management, consultation across specialties, and collaborative practicewith nursing, allied health, and community partners.You'llmanage a varied inpatient caseload, provide senior medical advice to cliniciansacross the hospital, support complex clinical decision-making, and work closelywith hospice and primary care teams to ensure seamless transitions for patientsbetween care settings.
Theteam consists of SMOs, CNSs, RMO and NP colleagues.
We work closely withhospital teams, primary care, community providers, and hospices to ensureequitable, patient-centred palliative care.
