About the Role
Your day will primarily involve contacting and visiting older people in their own home environments to determine their ability to maintain their independence.
This includes working with clients their whanau and the wider community support service agencies to ensure the client's care needs are met.
About the Team / Service / Location
You will work as part of our multi-disciplinary rehabilitation team to provide clinical assessment for older people with long-term conditions requiring support services to maintain their independence in the community.

About you
Registered Nurse / Registered Allied Health Professional
Commitment to achieving equitable health outcomes for Maori and vulnerable populations
Demonstrate ability to manage time and resources effectively to achieve positive outcomes.
Demonstrate ability to build and develop therapeutic working relationships, with our clients and their extended whanau.
Demonstrate a good knowledge of health issues experienced by ageing populations, including treatment and rehabilitation.
Full Driver's License
