About the Role
TheCommunity Rehabilitation Enablement Support Team (CREST) is at the forefront ofcommunity-based healthcare, working with clients to optimise safety andindependent functioning in their own homes.
As an Occupational Therapist in CRESTyou will be required to look at clients holistically but focus on the detailsto meet their safety and rehabilitation needs at home.
We have a full time position(1.0FTE) opportunity, 40 hours per week for a NZ RegisteredOccupational Therapist.
In this role, you will be responsible for :
Undertaking occupationaltherapy assessments and developing and implementing intervention andtreatment plans in conjunction with the client and whanau
Managing and prioritising abusy caseload
Carrying out assessments andtreatments plans in a holistic manner with a view to possible referral toother disciplines and support agencies
Troubleshooting barriers to discharge
Delegation to kaiawhina / AHA to ensure client needs are met in a timely and safe manner
Ensuring timely assessmentof patient needs on discharge to ensure optimum support is provided asthey return to their homes
About You
Ideally you will be an Occupational Therapist who is experienced in gerontology and / or rehabilitation, who is excited to work in a patient outcomes environment.

The ability to hit theground running with the energy and passion to deliver a high standard ofcare to patients
Flexibility and adaptabilityto work as a member of an inter-disciplinary team
Enthusiasm for knowledgesharing across and beyond your discipline
The ability to establish anearly rapport with patients and families, and interact with a wide range of people from varying backgrounds
Competency with te ao Maori,tikanga, and te reo Maori or a commitment to starting your journeyand taking ownership of your learning and growth
Good clerical, literacy and computer skills
Demonstrated specialty area experience and professional development is preferable
As this is a communityposition and travel is required, a full NZ Driver's license is essential
Registration with the NZOccupational Therapy Board and an Annual Practicing Certificate isessential
